Transitions in Care: Reduce Resident Risk for Adverse Events

The Centers for Medicare & Medicaid Services’ (CMS) proposed rule addresses transitions of care. CMS contends that during the transitions of care, residents are placed in vulnerable positions for adverse events and negative outcomes, and that effective communication is key to reducing risk factors for residents during transitions. The proposed requirement would necessitate additional admissions policies, discharge when there are safety concerns, and other items related to resident rights.
