Improve Care Transitions: Reassess When To Send Residents to the Emergency Department

Nursing homes face multiple pressures to send residents to the emergency department (ED) to mitigate the potential risks of adverse events such as falls, acknowledged panelists at the Sept. 15 National Provider Call (NPC) on the National Partnership to Improve Dementia Care and QAPI. However, providers can take steps that potentially could eliminate the need for this resident transfer, as well as possible hospital readmissions. ED visits and particularly hospital readmissions are targeted via several Centers for Medicare & Medicaid Services (CMS) quality initiatives, including the Five Star Quality Rating System on Nursing Home Compare, the Skilled Nursing Facility Quality Reporting Program (SNF QRP), and the SNF Value-Based Purchasing (SNF VBP) program.
