AADNS Facility-Wide Assessment Workbook

Beginning Nov. 28, 2017, as part of the Phase 2 Requirements of Participation regulation, skilled nursing facilities must “conduct and document a facility-wide assessment to determine what resources are necessary to care for its residents competently during both day-to-day operations and emergencies. The facility must review and update that assessment, as necessary, and at least annually. The facility must also review and update this assessment whenever there is, or the facility plans for, any change that would require a substantial modification to any part of this assessment.” 483.70(e)  

Use the AADNS Facility-Wide Assessment Workbook to:   

  • Help your facility comply with the new federal regulation
  • Identify areas where staff education and competency-based training needs to occur
  • Ensure the safety of residents during normal operations and during emergencies  

The AADNS Facility-Wide Assessment Workbook, a spreadsheet-based tool, guides the user in assembling the various subparts to comply with the regulation, including:  

Resident population

  • Both the resident census and facility’s resident capacity
  • Care required
  • Staff competencies
  • Physical environment
  • Ethnic/culture/religious factors, such as activity/food/nutrition services
Facility’s resources 

  • Physical structures and vehicles
  • Equipment non-medical/medical 
  • Services provided 
  • All personnel education level/ competencies r/t resident care
  • Contracts/Memos of understanding and third party agreements
  • Equipment access including after hours/emergencies
  • Health information technology resources 

Facility-based and community-based risk assessment

  • All-hazards approach
